This CPB has been revised to state that: (i) Allosource femoral head bone graft is considered medically necessary; (ii) Optium is considered a medically necessary allograft; (iii) OsteoAMP Select is considered not medically necessary; and (iv) the following are considered experimental and investigational: (a) Avive tendon wrap, (b) CartiMax allograft, (c) Cerament bone void filler, and (d) collagen-based bio-inductive implants (e.g., Regeneten) for repair of rotator cuff tears.
