This CPB is revised to add large congenital melanocytic nevi to the list of medically necessary indications for benign skin lesion removal. This CPB has been revised to state that removal of cutaneous and subcutaneous neurofibromas is considered medically necessary if criterion is met. A list of lesions that are considered premalignant or benign has been added to the appendix. Criteria for evaluating a skin lesion for likelihood of malignancy are added to the appendix.
