This CPB is revised to state that recombinant factor IX (IXINITY) is considered medically necessary for persons with hemophilia B who meet criteria for standard dose therapy or continuous prophylactic therapy. This CPB is revised to state that von Willebrand factor/coagulation factor VIII complex (Wilate) is considered medically necessary for prevention of excessive bleeding during and after minor and major surgery in persons with von Willebrand disease.
