This CPB is revised to state that Kovaltry (recombinant factor VIII) is considered medically necessary to prevent or treat hemorrhagic complications in adults and children with hemophilia A when criteria are met. This CPB is revised to state that Idelvion (recombinant factor IX fc fusion protein) is considered medically necessary for adults and children with hemophilia B when criteria are met. This CPB is revised to state that prothrombin complex concentrate (human) (Kcentra) is considered medically necessary for urgent reversal of acquired coagulation factor deficiency induced by vitamin K antagonist (e.g., warfarin) therapy in adults with the need for urgent surgery or other invasive procedure.
