This CPB has been revised to state that recombinant coagulation factor VIIa - jncw (Sevenfact) is considered medically necessary for the following when criteria are met: (i) hemophilia A with inhibitors; and (ii) hemophilia B with inhibitors. This CPB has been revised to state (i) recombinant human anti-thrombin III (ATryn) is considered medically necessary for the prevention of peri-operative (i.e., surgical procedures) thromboembolism and for the prevention of peri-partum (i.e., obstetrical procedures) thromboembolism; (ii) human anti-thrombin III (Thrombate III) is considered medically necessary for treatment of asparaginase-induced antithrombin deficiency in individuals who have had a thromboembolic event while on asparaginase therapy; or for prophylaxis when pre-thrombate III antithrombin III levels are below 60%.
