This CPB has been revised to state that (i) oxygen for home use is considered experimental and investigational for migraine headaches, and (ii) duplicate oxygen systems are considered convenience items and not medically necessary.  This CPB is revised to state that the reasonable useful lifetime of oxygen equipment is five years.
