This CPB has been revised to (i) state that intravenous iron therapy is considered experimental and investigational for the treatment of post-operative anemia following major surgery (e.g., cardiothoracic surgery, colorectal cancer surgery, and neurosurgery) and (ii) to add an iron deficiency status footnote to the medically necessary chemotherapy-induced anemia indication.
