This CPB has been revised to state that Triferic, a sodium ferric gluconate complex containing ferric pyrophosphate citrate, is considered medically necessary for the replacement of iron and maintain hemoglobin in hemodialysis-dependent chronic kidney disease members via dialysate during each dialysis treatment.
